Forms & Intake

See how OfficeHubb forms collect lead information and how enabled products and questions shape the intake experience.

What it is

OfficeHubb forms collect prospect information in a conversational flow instead of one long traditional form.

Your agency controls which products are offered and which questions are enabled.

What your agency can configure

  • Enabled insurance products
  • Carrier-related settings
  • Optional intake questions
  • Custom questions where supported

How forms connect to the rest of OfficeHubb

Completed form submissions can feed the pipeline and related team workflows automatically.

That means your team can move from intake to follow-up without re-entering the same information by hand.

Service requests and who gets them

Service work that comes in by phone, website chat or a website form (ID cards, billing, a new car, a claim, a lender change) lands on the Requests page, assigned to one person:

  • the person the client asked for by name, if they asked
  • otherwise the next person in your service rotation, or one person you chose to get them all

The button beside the search box on Requests shows who is up next. Owners and managers open it to set the rotation: who is in it and in what order, a switch per person, pausing someone until the day they are back, skipping a turn, or "Always one person". Front Desk, Settings, "Send them to" is the same choice. Agency AI can change it for you too.

Documents by email

With the AI phone line, the top of Front Desk shows where a caller who needs to send a document in is told to email it: the person next in line for service, at their own email, or your office's own inbox. Press Copy to give out the same address. Owners and managers change it under Front Desk, Settings, Where callers email documents. On the Requests page, the caller's request says "Waiting on a document by email to [name]" until it is marked done.

Common questions

Why do some questions appear for one product but not another? Question visibility depends on product selection and agency configuration.

Can we customize the intake experience? Yes. Agencies can configure enabled products and optional questions, while OfficeHubb-managed system questions remain protected.

Where do completed submissions go? They connect into your agency workflow, including the pipeline, according to the product's live setup.
